Is This Even “Real” Anxiety?

If you’ve wondered whether this even counts, since you’re still performing, still hitting deadlines, still showing up exactly the way everyone expects, that question is common, and worth answering directly instead of glossing over. High-functioning anxiety isn’t a formal diagnosis.

It’s a term the Anxiety and Depression Association of America (ADAA) uses to describe anxiety that shows up alongside achievement instead of in place of it. Clinically, what’s usually underneath it is generalized anxiety, sometimes layered with perfectionism or burnout, expressed through over-preparation, over-monitoring, and a body that stays braced even when nothing is actively wrong.

Staying functional doesn’t mean nothing is happening. It usually means you’ve gotten very good at carrying it without anyone noticing, including, some days, yourself.

Common Signs of High-Functioning Anxiety

Anxiety like this rarely starts as a thought you can argue with.

It starts as a body that won’t come down off alert: a racing heart before a routine email, a jaw that’s tight by 10am, a mind that keeps scanning for the next thing to handle.

The vigilance that helped you get ahead doesn’t know how to switch off once you have. The instinct to anticipate problems before they happen, useful in a meeting, becomes catastrophizing at 11pm.

A strong sense of responsibility makes rest feel like falling behind. It’s a system that adapted to a highpressure environment and never got the signal that it’s safe to stand down.

In the first session, we hear some version of the same thing from almost every client: “I don’t know why I can’t just relax, nothing is actually wrong.” If that sounds familiar, you’ve probably already ruled out the obvious explanations. There’s no crisis  and your body still hasn’t gotten the memo.

What you’re usually carrying is a nervous system that has been switched on for so long it no longer remembers what off feels like. We do not take away your drive, we help you stop relying on anxiety to sustain

If Anxiety Advice Has Never Quite Landed

If you’ve already tried therapy, an app, or a stack of self-help books for this and none of it really landed, that’s not a sign the anxiety isn’t real. It’s usually a sign the advice wasn’t built for you.

Most general anxiety guidance assumes anxiety that looks like avoidance, panic that’s visible from across the room, situations you’re visibly steering around. Yours probably doesn’t look like that.

It looks like overdelivering. Breathing exercises and “let it go” scripts tend to bounce off a mind that has spent years outworking every problem it’s faced. You don’t need to be told to relax. You need an approach built for how your mind actually operates, not a worksheet written for a general population

Our Approach

We use evidence-based approaches, adapted to a mind that’s usually already several steps ahead of standard advice.

The mix and pacing depend on what’s actually driving things for you, racing thoughts, physical tension, sleep that won’t come, not a fixed sequence applied the same way to everyone who walks in anxious.

Cognitive Behavioral Therapy (CBT)

identifies the specific automatic thoughts feeding the anxiety, the catastrophizing, the all-or-nothing thinking, and builds tools to interrupt them in real time, not just in the session room.

What Working With Uncover Looks Like

The first session starts with where the anxiety actually lives for you: your body, your sleep, the thoughts that loop, not a standard checklist. We listen to what you’ve already tried and what you want to feel like instead, then talk honestly about whether this is the right fit.

Early on, you may notice small shifts first, catching a spiral a little sooner, or getting through a hard day without it bleeding into the whole evening.

The bigger shift, trusting that you don’t have to stay on alert to stay capable, tends to build gradually rather than arrive all at once.

“Many high-achieving people aren’t struggling because they lack coping skills. They’re struggling because the strategies that once helped them succeed have become exhausting to maintain.” — Kristie Tse, LMHC-D

FAQs About High-Functioning Anxiety Therapy in NYC

Is high-functioning anxiety a real diagnosis?

Not on its own; it’s a descriptive term, not a formal DSM-5 diagnosis. Clinically, what’s usually underneath it is generalized anxiety, sometimes alongside perfectionism or burnout. The label matters less than whether what you’re feeling is actually affecting your life, even if no one else can see it.

Will therapy make me less driven?

No. The goal isn’t to lower your standards or talk you out of caring about your work. It’s to stop your nervous system from needing constant pressure to feel “on.” Most clients keep their ambition and lose the version of it that was costing them sleep.

How long does anxiety therapy take to work?

You may notice meaningful shifts within four to six weeks of consistent weekly sessions. Some people work with us for a focused few months; others continue longer. Progress sets the pace, not a fixed number of sessions

How much does anxiety therapy cost in NYC?

Uncover is a private-pay practice. The initial session starts at $350 and future sessions start at $300, depending on your therapist and their sliding scale availability. Exact fees are shared during your free consultation. Private pay means no insurance limitations, no diagnosis required for coverage purposes, and fully confidential records. You’ll receive

Is online therapy actually effective for anxiety?

Yes. Research consistently shows virtual therapy produces outcomes equivalent to in-person sessions for anxiety. For high-achieving professionals managing full calendars, it also removes a commute that often becomes one more thing to feel anxious about.

Do you accept insurance?

We’re out-of-network. We can submit claims on your behalf or provide a superbill so you can seek reimbursement directly, depending on your plan’s out-of-network benefits

How is this different from generic anxiety advice?

Most generic anxiety advice is built for someone whose anxiety looks like avoidance. If yours looks more like overworking, overpreparing, and overthinking, the standard scripts usually don’t hold up. We build the work around how your specific mind operates, not a worksheet made for a general population.
